Care Organizing Tools
The tools on this page help families keep track of important information about their child's health and care.
The Care Notebook is a 3-ring binder. It contains many types of forms that families may fill in. It also includes plastic pages that hold business cards and several pocket dividers that hold papers. The Care Notebook is only available in English for now.
The Care Organizer is a plastic file folder that expands. It has labeled pockets to help you organize your paperwork. It does not come with forms to fill out. Sections include:

- and more
It comes in English, Spanish, Vietnamese, Russian, Chinese and Somali.
How can these organizers help?
In caring for a child with special health needs, families may get information and paperwork from many sources. The Notebook or Organizer help organize information in a central place.
How Can I get a Care Notebook or Organizer ?
- Download or print the Care Notebook or individual forms (below).
- Order a Care Organizer or Notebook.
- Flyers that describe the items are also available for you to share with other families.

Families in Washington State may order one Care Notebook or one Care Organizer per child with special needs at no cost. Call Children's Resource Line and provide your name, mailing address, phone number and the item you would like to order.

Can Washington professionals order copies?
Yes, Washington professionals may order one sample copy of the Care Notebook and Care Organizer to use as tools to show parents. To order additional copies, Care Notebooks cost $10.00 each and Care Organizers cost $8.00 each. For quantity orders or questions, contact Patty Centioli at (206) 884-5735 or patty.centioli@seattlechildrens.org
What about orders outside of Washington State?
Current funding only supports the production of materials for families in Washington State. Therefore, for orders out-of-state, the Notebook and Organizer cost $20 each. Up to 5 of each Care tool may be ordered. Contact Patty Centioli for more information at (206) 884-5735 or patty.centioli@seattlechildrens.org.
