Care Organizing Tools

The tools on this page are organization systems to help you keep track of important information about your child's health and care. There are two kind of systems:

1.The Care Notebook is a 3-ring binder. It contains many types of forms that families may fill in. These forms include:

  • A medication list
  • Medical provider lists
  • Emergency information
  • Appointment calendar
  • Transportation plans
  • School check list
  • and more

It also includes plastic pages that hold business cards and several pocket dividers that hold papers. The Care Notebook is only available in English for now.

2.The Care Organizer is another kind of organization system. It is a plastic file folder that expands. It has labeled pockets to help you organize paperwork. It does not come with forms to fill out. Sections include:

  • Contacts
  • Appointments
  • Clinic visits
  • Medication
  • Therapy
  • Test Results
  • Resources
  • School
  • and more

It comes in English, Spanish, Vietnamese, Russian, Chinese and Somali.

How can these organizers help?
In caring for a child with special health needs, families may get information and paperwork from many sources. The Notebook or Organizer help organize information in a central place. Choose one based on your style of record keeping and/or how complex your child's condition is.

How Can I get a Care Notebook or Organizer ?

  • Download or print the Care Notebook or individual forms (below).
  • Order a Care Organizer or Notebook.
  • Flyers that describe the items are also available for you to share with other families.

If you have questions about any of these files or if you would like to adapt or reprint the Care Notebook, contact Patty Centioli at patty.centioli@seattlechildrens.org.
