A medical home is a coordinated and respectful health care partnership between a child, his or her parents and the child's primary care doctor. Parents and health care providers see the medical home as a place for sharing information about local resources, organizing care (how to organize care) for the child, and connecting with community-based programs. For more information about medical home visit the Washington State
Medical Home Leadership Network.
